Appeal & Grievance Forms

Grievance Form

The Grievance Policy exists to ensure a formal and transparent process for resolving grievances within the ASLTA and to promote a healthy environment for all ASLTA members.

The grievance policy only covers individuals who are ASLTA members. Non-members may only file a grievance in relation to unethical conduct of a member. To address an issue within the ASLTA, the member must first deal with the issue with the accused through a civil manner before following the official grievance procedure. All grievance processes will be conducted privately between both parties and those directly involved.

NOTE: A grievance may be filed only if an informal meeting with the person or party involved does not resolve the issue. Members are strongly encouraged to attempt to resolve the matter directly before filing a formal grievance. Include the date of the informal meeting and what was discussed on the form.

For full procedure, please refer to Chapter 6 in the Information Policy and Procedure Guidelines.

Appeal Form

If one or both parties are dissatisfied with the investigation progress and/or the final decisions, they may file an appeal. The appeal Google form must be completed and submitted within 15 calendar days of the resolution (see Appendix 9) . The appeal process will follow the same process as filing a grievance.
